Physical & Inorganic Chemistry Safety Quiz Instructions and Information:

In order to be enrolled in Physical & Inorganic lab courses, you must pass the online safety quiz.
The quiz consists of 25 multiple choice questions.
You will be provided with three opportunities to pass the safety quiz (once you have passed the quiz you do not have to take it again).
In order to pass the safety quiz during the first time it is administered, you must obtain a minimum passing score of 80% (20/25).
In order to pass the safety quiz during the second time it is administered, you must obtain a minimum passing score of 84% (21/25).
In order to pass the safety quiz during the third time it is administered, you must obtain a minimum passing score of 88% (22/25).
Failure of the quiz three times means that you must drop the class.
Here is the schedule of each safety quiz:
Round 2: Friday, April 4, 5:00 PM -- Tuesday, April 8, 5:00 PM (84% correct is passing)
Round 3: Tuesday, April 8, 5:00 PM -- Thursday, April 10, 12:00 PM (88% correct is passing)

It is your responsibility to take and pass the
quiz. No "make-up" quizzes will be given.
You will have 30 minutes to complete the quiz, therefore make sure that you have 30 minutes of uninterrupted time. After 30 minutes, the quiz will be terminated and your will receive the score at the time of termination.
Multiple people taking the quiz on the same computer at the same time will lead to unexpected (bad) results.
Answers for this quiz are to be your own. Once you have started the quiz, you are expected to work without assistance from others.
Once you choose to answer a question, you may not change your answer. However, you may look at a question as much as you would like prior to answering.
During the quiz you will be presented with your current score. Make sure you have passed the quiz, if you have not, then be prepared to take it the next scheduled time. You do not need to report your quiz score, it is automatically collected.
QUIZ LOGIN INFORMATION: For the Quiz you will be prompted for an "ID
Number" and "Password": your ID Number is your student "W" number without the
"W" or the numbers after the dash; your Password is the last two digits of
your ID Number combined with the first two letters of your last name (for
example Joe Smith with ID Number 12345678 would have a password 78Sm).
